Immunisation Requirements

When enrolling your child at Starbright you are required to provide your child’s Australian Immunisation Register (AIR) Immunisation History Statement. This statement must not be more than two months old. Only parents of children who are fully immunised or are on a recognised catch up schedule can receive the Child Care Subsidy and the Family Tax Benefit Part A end of year supplement. You will receive your child’s Immunisation History Statement in the mail after their 18-month and 3 1/2 - 4 year old vaccinations. You can request a copy of your child's immunisation details at any time by:

If your child is not immunised and you want an immunisation exemption, you need to provide the Department of Human Services with one of the following from your immunisation provider or doctor:

  • An Immunisation exemption for medical reasons (contraindication) form
  • An Immunisation exemption by conscientious objection form, or
  • A letter explaining why the exemption is necessary
